Middle School Athletics
Our fall athletic season begins soon with basketball! We also offer four other sports later in the school year: volleyball, track, soccer, and flag football, all for boys and girls. Now is a great time for your student-athletes to submit the required FHSAA paperwork to tryout and play online.
All paperwork is submitted at www.planeths.com. No paper copies are accepted at Rampello as all clearances must be done online. 7th and 8th graders must have had a 2.0 GPA in the spring semester to be academically eligible. All middle school student athletes will have GPAs reviewed in the spring for academic eligbility.
Some paperwork you will need to upload that can be scanned on your phone include:
- A brand new EL2 physical form from FHSAA. We recommend seeing your child's pediatrician to have them complete this as they may have the information and form already. This must be signed, completed, and dated by a medical provider.
- Three required FHSAA videos. The required courses are: Concussion for Students, Sudden Cardiac Arrest, and Heat Illness Prevention. These must be done in the STUDENT'S name, not the PARENT'S name. Make this your student's homework have them watch these now before school starts! Screenshot or download the final certificates to upload to the PlanetHS account.
- District athletic insurance. Every student-athlete needs this before stepping on to the field for tryouts. Upload the paper "card" PDF you receive after you purchase the insurance.
- Government Issued Photo Identification of parent/guardian who is signing the forms for the student-athlete. Make sure the ID you upload matches the parent name who is digitally signing the paperwork.

Basketball Season Info:
All basketball players must be cleared by Friday August 18 in Planet HS. Please upload paperwork early so corrections can be made if necessary!
Basketball tryouts will be the week of August 20. More info will be coming soon!

7th Grade Immunization Requirements
To enter the 7th grade in Florida schools, updated immunization documentation is required BEFORE entry on the first day. You can see your child's pediatrician for copies of the appropriate Florida Department of Health form or visit a Back 2 School Health Clinic at www.back2schoolhealthclinic.org.
Updated immunizations can be sent via email to our school nurse at michelle.hinson-williams@hcps.net or faxed to us at 813-233-2337. Please see the state requirements below.
Students entering seventh grade must submit an updated immunization record with a Tdap. All students from seventh through twelfth grade immunization record must that the student has met the minimal state requirements:
- 5 doses DTaP (diphtheria-tetanus-pertussis)
- 4 doses Polio (IPV or OPV)
- 2 doses MMR< (measles-mumps-rubella)
- 3 doses Hepatitis B
- 1 dose Tdap (tetanus, diphtheria, pertussis)*
- 2 doses Varicella (chickenpox) or has had the disease as documented by a healthcare provider**
*Note: 1 dose Tdap (tetanus- diphtheria- pertussis) for seventh grade is required. An updated DH 680 form to include Tdap must be obtained for submission to the school.
**Varicella vaccine is not required if Varicella disease is documented (the year the child had the disease must be included) by a Licensed Physician, Advanced Registered Nurse Practitioner or Physician Assistant.

Attendance
There are 180 school days in a calendar year. It is now more than ever to ensure that your child is at school on time, for the entire duration for school. Our students learn so much in one day and unfortunately, we are still closing our achievement gap that was created from COVID.
Please view the attached graphic with information about how important good attendance is for your student. Contact our school social worker, Mrs. Counce, if you have any questions or concerns regarding your student's attendance! We would love to help any way we can in helping your student be successful at school!
