Healthy Student Forms

This is NOT an application for insurance.  This is so clinic nurses can give your student over the counter medications like Advil, etc. for minor complaints.  If you do not have a form on file, please download the Healthy Student Form and turn in to the clinic and/or the office.

Emergency Cards

If you have not returned a signed emergency card, please do so ASAP. These are important for signing students out to authorized persons, communicating important information, and reaching someone in the unlikely event of an emergency. Blank emergency cards can be obtained through the Student Affairs Office or downloaded here.
