The following mandatory forms and items are required to be completed and submitted for registration of a student. NO student can be enrolled prior to receipt of all forms. Please review the appropriate Registration Letter pertaining to your situation.
Please bring the following forms with you to the Registrar, Jocelyne Melendez, when registering your student.
Please note, that if you are enrolling from out of the country, we will have to submit copies of all ORIGINAL school transcripts and birth certificates to the District Bilingual Department for review and transcription. This may take several days to a week and NO student may be enrolled prior to this being completed.
