The following mandatory forms and items are required to be completed and submitted for registration of a student. NO student can be enrolled prior to receipt of all. Please revew the appropriate Registration Letter pertaining to your situation.

Please note, that if you are enrolling from out of the country, we will have to submit copies of all ORIGINAL school transcripts and birth certificates to the District Bilingual Department for review and transcription. This may take several days to a week and NO student may be enrolled prior to this being completed.

Side A - Complete Side A if you are the homeowner or lessee of the property, OR if you are voluntarily living with a family or friend (this is a Co-Residing situation, NOT due to a natural disaster, evition, or economic hardship (loss of income/job, or if you are temporarily living there while building a home, etc...) In this instance the person with whom you live and who must provide residency verificaitons will need to come to the school with you for administration approval.
Side B - Complete Side B if you are living with another person due to one of the hardships listed in Question 1 on this form. If you have any questions regarding your situation, please feel free to call the Counseling Deparment and ask for the Data Processor the Registrar, or the Secretary for assistance.
