Special Diets and Allegies
Student Nutrition Services takes food allergies, food safety, and health seriously. If your child needs a menu change or special diet due to a medical condition, food allergy, or for cultural or religious reasons, there are forms to submit. Here’s a breakdown of the necessary steps:
-
For food allergies or medical conditions:
- Complete the SNS Special Diet Packet Letter & Form 2024-2025 (Spanish form)
- Submit this form to both the school nutrition manager and school nurse.
-
For food intolerance or cultural/religious dietary preferences:
- Complete the Meal Preference Form (Spanish form) for 2024-2025.
- Again, submit this form to the nutrition manager and school nurse.
-
To stop or change an existing special diet:
- Fill out the Discontinuation of Diet Prescription Form (Spanish form) for 2024-2025.
A new form needs to be completed each school year, and submitting it is crucial for ensuring your child receives the appropriate meals that meet their needs.
It is important that the Diet Prescription for Special Meals Form is completed correctly and given to the Student Nutrition Manager and School Nurse at your site so we can safely serve your child. Food substitutions and/or menu modifications cannot be made without a completed form. A new form must be completed each school year so our records are kept up to date and the information on file is correct.
