Special Notes:

**Court documents or Caregivers affidavits are required for students living with someone other than their parents or legal guardians.

**If sharing a home by choice or hardship, the person with whom you reside must sign the residency form in addition to the parents or legal guardian.

**All registration documentation must be received for your student's registration to be completed.

**If any other special circumstances apply, please ask to speak to the Registrar or Social Worker in private.
