See the English/Spanish forms below.  Due to confusion regarding the previous consent for health services forms, Hillsborough Public Schools has developed the attached PARENT/GUARDIAN WITHHOLD/DECLINE CONSENT FOR SCHOOL HEALTH SERVICES form. This form will supersede all previous versions.

THIS FORM SHOULD ONLY BE RETURNED IF YOU WISH TO DECLINE THE SERVICES LISTED ON THE FORM.

This form is not to be confused with 6^th Grade Mandated Screenings. A notification with instructions was sent home for that screening.
