Once you have entered all required information, email both forms AND documents from checklist (birth certificate, health records, 2 proofs of address) to: carmen.brinda@hcps.net 

Please format documents into PDF attachments before emailing. (If using an Iphone you can open the "Notes" app; the scan button automatically saves documents as a PDF)
