This page is intended to help you with the registration process. You are welcome to print out the pages and fill them out if you have access to a printer. However, copies will be available upon arrival to the school's main office.
A description of each document has been provided. Please note: the registration card has been provided for your information only. The original document will be completed by the student's parent/legal guardian during the registration process.

Parent Verification of Student Status
This form provides us with the information we need to ensure the student has a successful start and welcome to our school.

State Mandated Information for Mental Health
Per Florida Senate Bill 7026, this information is required when enrolling students.

Request of Records
This form will be sent to the school your student(s) is(are) coming from before Tomlin. It is unnecessary for you to sent this to the previous school. We will need to know the school the student previoulsy attended.

Complete the Engligh or Spanish version of this form. Be sure to bring 2 of the required documents that verify residence.
